应用SPECT研究头皮穴区局部浸润麻醉后对电针信号传导的影响王 凡 郭长春 贾少微


  [摘 要] 目的:探讨和验证腧穴和神经系统的关系。方法:选择头皮运动区上1/5和中2/5处针刺,观察1小时之内连续接受针刺前、电针和局部浸润麻醉后电针刺激3次SPECT脑灌注显像。结果:针刺前见双侧大脑皮层、丘脑、基底节和小脑的血流灌注和功能基本对称;当电针头皮运动区时,以对侧丘脑、对侧额叶、对侧顶叶的运动和感觉皮质区增高更为明显;而当局部浸润麻醉后再电针时,对侧丘脑血流灌注和功能下降。定量分析结果和视觉所见相同。结论:①一侧头皮针刺激可以同时改善双侧局部脑血流灌注和功能活动;②研究结果支持头皮针刺信号是通过三叉神经传导径路入脑并发挥效应;③依据SPECT研究结果尚看不出头穴分区与大脑皮层各功能区存在相应的定位关系;④当穴区被局部浸润麻醉后,针刺信号的效应明显减弱或被阻断。

  [Abstract] Purpose

Toexploreandtestandverifytherelationbetweenacupointsandthenervoussy

stem.Methods

Acupuncturewasgivenatupper1/5andmiddle2/5ofthemotorareaofscalp,

andeachpatientreceived3timesofSPECTcerebralperfusiondevelopmentbefo

reacupuncture,

duringelectroacupunctureandelectroacupuncturestimulationafterlocali

nfiltrationanesthesiawithin1hour.Results

Beforeacupuncturethebloodperfusionandthefunctionatbilat-eralcerebra

lcortex,thalamus,

basicalganglionandcerebellumwerebasicallysymmetric;

atelectroacupunctureofthemo-torarea,

themotorandsensorcortexareasofcontralateralthalamus,

frontallobeandparietallobeincreasedmoreobvious-ly;

acupunctureafterlocalinfiltrationanesthesiadecreasedthebloodperfusi

onandfunctionofcontralateralthalamus.Quantitativeanalysisandfinding

ofvisionhadsameresults.Conclusion ①

  许多研究表明针刺时人体产生的"针感"与神经系统的完整性有关,神经干被切断或被麻醉药阻滞后镇痛效应消失[1]。
